Personal, - Beware of postpartum disorders

Although delivery is a physiological process that is experienced by all expecting females, there comes with it a considerable change in the biological, psychological and social balance. Sometimes this change is akin to an earthquake in the life of the woman if it is not preceded, accompanied and followed by enough health care - psychological and social. The degrees of that effect on women vary from one person to another. We see that some women pass this time smoothly and simply giving an impression that delivery is a natural physiological change. On the other hand, we also notice the indications of a real crisis which precedes, accompanies or follows the delivery in other cases.
It is very important to know the limits and types of postpartum disorder through doctors or even through common people. People who suffer from some of these disorders simply need to be assured, while other disorders require the patient to be admitted to hospital.
In this article, Dr. Muhammad Mahdi, a psychiatric consultant, discusses the indications of these disorders.
Postpartum Depression
This is considered a state of illness and occurs among 50% of the women after delivery. This case begins a few days after delivery and lasts for some days, and is characterized by having a state of depression, a bad mood, frequent crying, feelings of fatigue, worry, angry outbursts and a desire to depend heavily upon others. The severity of these symptoms decreases with time and the mother then goes back to her normal state. This is due to rapid hormonal changes, the pressure of delivery and the woman’s awareness of the increase of her responsibilities which motherhood necessitates. This case requires nothing except assuring the mother and psychologically and socially supporting her until she is over these feelings.
Postpartum Psychosis
She was a student at the faculty of Law and she knew an engineer whom she insisted on marrying in spite of her family’s refusal due to the social differences and the notoriety of this man. The girl underwent a state of depression during the engagement. The family of the girl attributed this to the problems which accompanied the engagement. This state lasted for about two months and ended with the use of some folk medicine. When the girl married, she found that her husband, whom she had preferred to her family, was treating her very badly. He would leave her alone all the time crying over her misfortune. When he came home, he did not do anything but neglect and insult her. In the meantime, none of her relatives visited her because the tension of the relation between them and the husband. When she became pregnant, she suffered from continuous vomiting, inability to undertake her household chores, loss of appetite, continuous insomnia, and severe deterioration of her health. When she asked him to allow her mother to stay with her for some time until she would get better, he totally refused. Accordingly, she tolerated these conditions unwillingly until she gave birth to her child.
Two weeks after the delivery, her mother-in-law noticed that she was in a strange state; she was frequently absent minded. She would sit for long durations of time without moving and then become very worried and nervous without any clear justification. She stopped eating and sleeping at night or day; sometimes she cried and at other times she screamed. There was no one beside her at that time except her mother-in-law for her husband insisted on preventing her family from visiting her. Her husband tried to help her by bringing some folk healers, yet her state became worse. They noticed that she looked angrily at her child throwing him about violently from her arms. At this point, her mother-in-law took the child in order to protect him from his disturbed mother.
Her mother-in-law went to the market to buy some things and left the child in a room beside the mother’s room. When the mother-in-law returned from the market, she found that the mother had taken the child to the kitchen and slaughtered him with a knife.
I apologize to the reader if this incident was terrifying, but this was a real case which I saw and received at the hospital myself when he was about to die. This mother was arrested after the incident and detained for some days in order to investigate the crime. During these days, everybody was disgusted with her because of her atrocious act. No one can bear seeing someone killing an innocent child and therefore she felt alienated and disgusted among her prison inmates and from many of the supervisors in the prison. She stopped eating and drinking or was perhaps prevented from doing so by others due to them thinking that she was a criminal who did what she did intentionally. She was in a very bad condition and was then moved to a hospital where she received many physical and psychological treatments until she finally got better. This was an example of postpartum psychosis. This disorder requires the attention of gynecologists, a family counselor, psychiatrists and those who need to be around the patient due to the dangerous nature of this case.
The occurrence of this disorder ends during the first year, after about two or three weeks after delivery. The longest expected time for this condition is eight weeks after delivery. It is thought that this disorder takes place due to one or all of the following factors:
- Pressure of Delivery
There is no doubt that delivery causes psychological pressure; it is not only because of the pain and difficulty of delivery, but the mother also suffers throughout the months of pregnancy and then reaches its climax at the point of delivery. She spends many sleepless and uncomfortable nights. Then, she is surprised to find a child beside her who needs care. She spends the night awake and his needs are endless. These pressures are intensified if she does not have a natural delivery, such as a caesarean or complicated delivery.
- Rapid Hormonal Changes
After delivery, there is a rapid decrease in the level of estrogen and progesterone and this unbalances the biological state for some time.
- Psycho-Social Factors
This occurs if the woman lives in a disturbed relation with her husband or his family, undergoes pressure at work or suffers from previous psychological disorders that were not successfully treated. Sometimes, the mother refuses pregnancy and accordingly rejects the child because she does not want to become a mother due to the psychological problems that she has, or due to getting married to someone who she does not feel comfortable with or due to her fear of the responsibilities of motherhood. Some researches have discovered that marital disputes during pregnancy increase the risk of being subjected to postpartum psychosis.
- Certain Biological Factors
Such as pre-eclampsia or taking certain medications. This illness is preceded by some inceptive symptoms, such as, insomnia, discomfort, restlessness, angry outbursts and inability to concentrate. Then, the symptoms appear, such as severe depression, hypomania, awareness disorders, absence of concentration, disorder of realization, memory and thinking, the appearance of some aberrations such as thinking that she is still a virgin or that she did not give birth and that the child is not her own or that he is a devil. She may also sometimes get hallucinations like hearing a sound telling her that she should kill herself or her child to relieve him from the suffering of the this life, and disorders in moving. These symptoms are mostly accompanied by confusion of consciousness, doubts and obsessions over the well-being and health of the child.
This matter should be taken seriously when the case is diagnosed since it is a psychological emergency. It is important to place the patient in a mental hospital if her psychological health deteriorates or if she is inclined to commit suicide or to harm her child. The child should be placed under the care of one of the relatives and the mother should be allowed to see him every now and then under complete supervision so that she does not have a chance to harm her child. These cases can be successfully treated through anti-depressants and lithium; sometimes, the antis of psychosis are added. In severe cases, electroconvulsive therapy )ECT( is used. After the elimination of the severe symptoms, the patient can be helped through psychological treatment where the psychological and social problems are discovered, and then she is psychologically supported and helped to accept her new role as a mother.

Personal, - A woman's color and preterm delivery

After British researchers had studied birth records of more than 36,000 babies born in the north of the City of Birmingham for the period of four years, they discovered that dark skinned and colored women were more liable to have preterm delivery and birth of premature babies than white women. Three thousand births, meaning, 8 percent of premature infants, who were born at least three weeks ahead of their due birth date, were from mothers of African origins.
After determining factors of age and social status of mothers, researchers found that 22 percent of African women are more liable to give birth before the duration of pregnancy )which is almost 37 weeks( is complete; 78 percent give birth before the thirty-fourth week of pregnancy; ladies who give birth to premature babies before 28 weeks of pregnancy are fourfold.
The researchers, according to the International Journal of Gynecology & Obstetrics, see that social deprivation represents a factor that increases the danger of preterm delivery. They point out that black children grow faster than their white peers, something that causes shorter-term pregnancies in their mothers.

During a casual conversation, a woman said to her neighbor, “You really have gained considerable weight. Watch out for yourself.” On hearing this, this second woman was deeply grieved and as annoyed as if she had heard the news of a relative’s death. She asked her peers worriedly, “Help me, what should I do? I have gained weight, how can I lose it?” That was a good question which was answered by the attending women with great zeal and interest. Here are some of the most important solutions that were mentioned before the poor, bewildered woman:
1-Joining a sports center to take part in different types of sports like swimming, running and exercising.
2-Following a specific diet in which one should not eat a lot of sugar and carbohydrates. A woman present even suggested that if she could not avoid eating many types of food, she could eat what she wants then vomit it out!
3-Taking some herbs, even though they may taste bitter and are hard to swallow –as, it seemed, true bitterness lies in the loss of slenderness and the difficulty to attain it.
4-Undergoing liposuction in a clinic or private hospitals to get rid of excess fat.
5-Having a gastric balloon at a hospital to feel that she is always full and thus, loses her appetite for food.
6-A step that guarantees success: to tie your mouth. Yes, go to a private clinic and pay about one thousand Saudi riyals to have your mouth tightened. You will only have a small opening in your mouth that would hardly allow liquids in through a tube. Only then, you will be forced not to eat and will lose weight in a short time. Moreover, your words will not be clear; no problem, patience is the way to relief. Be patient and you will have good results!
7-There is another new step which is to have half of your stomach stitched so that it will be have the capacity of a child's stomach. Hence, you will only eat a little.
These were the suggested solutions for the poor lady and these were their valuable comments. If you had only seen their zeal and excitement, you would have thought that the matter was far greater than this. However, it was only for the sake of being slender. All this discussion was meant for the sake of seeking beauty! If this is so, what would be the case of the one who seeks safety and recovery from a disease that has afflicted him? What would be the case of the one who pays all he has to get food to satisfy his hunger? What would be the case of the one who works day and night to save the price of a can of milk for his infant? What would be the case of the one who travels abroad to collect money to save the price of medicine?
Is excess leisure the reason behind this state that women have reached or is it love of appearance and clinging to this worldly life? Who is responsible for this: their upbringing, the media or something else?
Sister, if you truly seek to be slender, observe the following instructions:
1-Adhere to the prophetic advice regarding eating:“One third for food, one third for drink and one third for breathing.”
2-Do your housework by yourself and dispense with the maid if it is possible. Otherwise, help the maid with the housework and do not entrust all the tasks to her.
3-Fast frequently bearing in mind that your intention is to seek the reward of Allaah The Almighty.
However, you should not approach sports or physiotherapy centers. You will not lose any weight other than that of your wallet. With regard to inserting a balloon in your stomach, I warn you that it may burst inside you. Please do not tie your mouth because the world is in of your truthful words which call to goodness. If you give precedence to your slenderness over calling to the truth, then, you are giving the people of falsehood a chance, and silencing the truth by yourself.

Fathwa, - {Misconceptions about Muslim Women}, - Claims about woman in Islam

Question
There is a new group which has come recently to my city and claim that they are addressing common misconceptions about Islam by Muslims themselves.
These are a few of their teachings:
- looking at a woman in her eye while addressing her is a cultural thing and has nothing to do with Islam.
- women are highly encouraged to work )even outside homes, i.e. offices, etc.(.
- the public is Mahram for a woman.
- women only need husband's permission to go out only if the travel or period of absence is more than 3 days.
-the voice of a woman is not Awrah.
- males are allowed to attend and listen to Qur'an recitation competitions involving women.
- the seating arrangements which seek to segregate man and women is a cultural thing.
- Islam does not preach strict segregation of the sexes )they used Hajj as an example(.
- women are allowed to give Adhan )without any specifications(. However there would be grave consequences and that is why they don't do so.
- Sahabah used to sit in a big room with man facing women )more of a mixed seating but women with women and man with man, however in an open manner(.
- if they sat together)man and woman(, every woman sat with her Mahram in a big circle.
Please tell me if these things are right as I have had a different upbringing )different teachings(. If they are, give me a few evidence and if they aren't, again give me some evidences.

Praise be to Allah, the Lord of the World; and may His blessings and peace be upon our Prophet Muhammad and upon all his Family and Companions.
Men should not look at alien women nor should women look at alien men as this is forbidden by the clear texts of the Qur'an and Hadith. Allah Says )interpretation of meaning(:}Tell the believing men to lower their gaze )from looking at forbidden things(, … And tell the believing women to lower their gaze )from looking at forbidden things(, …{]24: 30-31[.
The Prophet )Sallallahu Alaihi wa Sallam( said:"Allah has written forAdam's son his share of Zina which he commits inevitably. The Zina of the eyes is the sight )looking at an alien woman("]Reported by Imamsal-BukhariandMuslim[.
The Prophet )Sallallahu Alaihi wa Sallam( said toAli:"OAli! Do not follow an unintentional look )at alien women( with another look. The first glance is for you )pardonable( whereas the other glance is not for you )that is a sin(". Transmitted byal-Tirmiziwho considered it as good. There are many Ahadith proving this ruling.
So, whoever claims that lowering gaze from looking at alien women is merely a cultural matter and not a religious act should recheck his religion, Islam.
2( Working is permissible for the woman as long as the following conditions are met:
a( When the Muslim community needs her services such as a lady doctor, lady teacher, etc.
b( Working in a non-mixed environment.
c( Observing the rulings of Sharia as for as the matter of Hijab is concerned.
Yet it is more suitable for her to stay in her house taking care of her children, the future generation, which is, indeed, best for her and for the community as well.
Allah Says )interpretation of meaning(:}And stay in your houses, and do not display yourselves like that of the times of ignorance, …{]33: 33[.
3( Jurists have different opinions concerning the matter of presence of a woman among a great number of men, is it permissible or not? The preponderant opinion is that it is permissible provided there is no temptation; otherwise, it is forbidden as agreed by all jurists.
However, traveling of a woman in public without being accompanied by a Mahram is not permitted with the exception of the dire necessity. Some jurists permitted her only to travel to perform obligatory Umrah and Hajj with the company of trustworthy group of women.
4( Going out without having husband's permission is not permissible, let alone traveling, which is strictly forbidden with a greater reason.
5( Is the voice of a woman Awrah )must be covered( or not? This is a matter of discussion among scholars. The preponderant opinion is that it is not Awrah except if there is a chance of temptation.
6( There is no harm if a man attends the Qur'an recitation competition of women provided the women are in complete Hijab fulfilling the rulings of Sharia and provided he is certain to not to be tempted on hearing their recitation.
However, it is better that only women attend in such competition in order to block the ways that could lead to evil.
7( Islam urges that there be separate places for men and women at gatherings since mixing of both sexes leads to many evil and shameful acts. There are many evidence proving this fact.
Allah Says )interpretation of meaning(:}And stay in your houses, …{]33: 33[.
The Prophet )Sallallahu Alaihi wa Sallam( said:"The best rows for men are the first rows, and the worst one are the last ones, and the best rows for women are the last ones and the worst ones for them are the first ones".
This is because the first row of women is closer to the men's ranks and the last rank is farther from them.
The Prophet )Sallallahu Alaihi wa Sallam( used to order men to delay leaving of mosque till the women left. This rule was only to prevent them from mixing with women. If it is undesirable for men and women to be close at the time of worshipping Allah, remembering Him, how much more so in a place full of doubts and temptations. The Prophet )Sallallahu Alaihi wa Sallam( said toUmm Salamah:"Perform the Tawaf while riding from behind the people"]Reported by Imamal-Bukhari[.
Hafiz Ibn Hajarhighlighted the reason and said:'The Prophet )Sallallahu Alaihi wa Sallam( ordered her to make Tawaf from behind the people since it would provide more cover for her from being seen by men'.
In short, we mentioned but a few evidences here; otherwise, the evidence is too much to be covered in this Fatwa.
8( Taking Hajj as an example for the permissibility of mixing men and women is totally baseless. Is it correct to say that wine is permissible since some Muslims drink it? It is not sound to take the example of Hajj to allow the mixing of both sexes since a mistake can never be an excuse for another mistake.
9( Women can call for Azan but without raising their voice. Imanal-Shafi'esaid:'Calling Azan and Iqama is permitted for women but it is better not to raise their voice'. The reason is that her voice may cause seduction to others.
10( As for their claim that the companions of the Prophet )Sallallahu Alaihi wa Sallam( used to sit in a big room facing women … we ask them to produce the evidence for this claim. This claim is full of ambiguity and is beyond the reality.
No doubt, some companions of the Prophet )Sallallahu Alaihi wa Sallam( used to visit other companions and sometimes some of them visit some women for some legal purpose but it was totally free from being alone with them, unveiled and free from any kind of suspicion, which is common now.
In short, their claims consist of true and false. It is apparent that they are keen to encourage men to mix with the opposite sex and socializing with alien women. Indeed, this practice will open many doors to seduction. Therefore, it is better for men to avoid being seen by women and it is better too for women to be far from the eyes of men as reported from Ayisha )Radiya Allahu Anha(.
We ask Allah to make us steadfast on right path.
